Why does my statement look different?
Our office has transitioned to a new practice management and payment system. You will notice the layout of our statements has changed.
How do I make a payment on my balance?
To make a payment on your balance, you will need your account number, which is located in the top right hand corner of your statement under, “ACCOUNT NO.”. After closing this pop up window, you can fill in the required information to submit your payment.

Patient Portal:
You can view and make payments on your account balance, upload insurance and ID cards, view and download clinical summary documents, send secure messages to your provider, and more from your patient portal.

If you would like to get access to your patient portal, please call our office at 360-733-4800 and choose option 3.
